Not saying this is the reason but it could be the first time the cock gets a bit of trauma, getting slammed in and out and maybe some teeth scrapes. This all happens when it is erect, then it shrinks and starts to heal. If it them becomes erect again it may cause some more micro-trauma to it which is painful.

You may need to do some testing to validate this theory, on one day do it twice in a row with a loose pussy lady, then on another day do it twice in a row with a tight pussy lady and compare cock soreness between the two.
