It's similar to holding pee for 3 hours, if by needing to pee you mean wanting to fuck everything in sight and making horrible life decisions.
I can confirm this. I travelled camping in a panel van with a hot girl I met who was also travelling. We canoodled dfk and petting in the back for a week, but she wouldn't give over. To answer the question, no real pain but I feel my testicles are 'full' and if I've been sitting a long time they sometimes start to ache.
The annoying thing is the wet dreams - having to wake up in the middle of the night to clean myself up, or even right before the alarm goes off. The really annoying thing is if I'm 'saving' myself for a punt later on the week and I explode in my pants two days out like last night.
Blue balls are more a result of being aroused and not being able to unload, rather than from just sitting around doing nothing.
