It sounds like you're going to a Caucasian shop.

Generally the girl will come to the waiting room, introduce herself and then ask you what you're looking for. You can say whatever you like and then she'll respond with whether or not she does it and then how much.

I'd just bring another $150 if you're interested in having the experience you're after. To be fair, you could probably go to an Asian shop to get the experience you're after without worrying about extras.

Never heard of a shop doing this. In Blacktown? I thought both Forge St shops and Carnegie pl had set prices. I know 10 Vangelli recently changed their price structure but didn't think it was a negotiated fee type thing (and they seem to be pricing themselves rather high too, they'd want to have the girls to match!)

I agree with Double_Adapter, provide a specification and call for written tenders. That way you can get the correct hole size you want.
