I don't think we have enough detail (we'd need mind reading for that) to know exactly what went on there, but there are clearly some facts.
1) Kay physically couldn't take any more.
2) No means no, good on you Speech writer for adhering to that.
3) Cutting the service is a huge no-no, and if Kay was truly unable to continue, refunding the 10(-20)mins worth should be adequate.
Judging from the wording I've seen, I am personally leaning more toward Speechwriter demanding more from the WLs than most of us, though even that does not excuse cut service. If Kay were offended/pushed/etc enough by Speech's needs, she could speak up and request not to go through with the service, full refund and perhaps simply book another girl more to his standards. Why didn't she? Either she was intimidated (doesn't particularly sound like it, from his account at least), or she changed her mind far too late. She should always have the option, of course, as her safety should be paramount, but it seems she had no intent until right near the end she switched completely.

In any case, that's just my two cents. Bit of a grey area, though I am leaning toward Speech's needs being a bit much for Kay and she changed her mind near the end.
