I've been experiment going to a proper Thai massage shop for $90 an hour, get a good massage for full 60 mins then popping over the Ginza for a 30 mins to finish off. usually this adds up to $270-$280.
A one hour massage plus FS is $290 and you only get 35mins of massage and 10 mins of FS....
Think about it boys.

I control the session 100%. Massage in my session is only max 10min. [ and that is, if I see the ml for first time] just for warm up and to know each other, then that is the path ... Tip: for 30min. 120$ - for 45min. 150$ - for 60min. 180$ ... plus the shop money 55-65-75.
-Can be maybe 20$ more but it depends how the session [ I am talking about 'Ginza service'] will go with the ml.
-Very few times [ only three / four times] ml disagrees with my tip, so in those cases I had only massage.
-95% of my sessions were / are almost 10 out of 10.
Therefore, almost never I go to shops that have inclusive rates, you know this 100-120-140, complicated staff is happening with this kind of set up most of the times.
