Take undies off. Groan when they come close to brushing your nut sack will usually give them the sign that you're after more and they will usually then start some tickle tackle. If not just ask them if they do extras. A lot of it depends on where you're going. Places like Flora or UC or Aitcheson St are guaranteed R&T so it's going to happen so you can start to touch the girl during the massage. Places like Princess Thai etc are more upscale so you may need to bring up the extras chat.
